My name is Seth Gaynes, founder of With Heart Private Care Consulting. I help families navigate private home care by connecting them with trusted, licensed caregiving agencies throughout Long Island and NYC.  When a loved one needs support after a hospitalization, rehab stay, dementia diagnosis, or increased fall risk, I help families coordinate the right care solution based on their needs, schedule, personality fit, and level of support required.  I work closely with families, elder care professionals, assisted living communities, and discharge planners to simplify what can often feel like an overwhelming process while remaining hands-on throughout the transition to help ensure communication and continuity of care.  My goal is to help families find the right care at the right time with compassion, clarity, and personal attention.
